Explanation:

Sometimes if the listhesis is bad enough, you will need surgery even if you’re not having a lot of symptoms because essentially you are guaranteed to get symptoms later on. Here is one that’s a little more subtle, but you can still see this comes down and then there is a step-off back to here. This is a grade I, this doesn’t necessarily need surgery if it’s a grade I, but it this slips 50% or more, then you have to fuse it, and can actually slide all the way out the front, and the operations to fix that are associated with a high degree of sexual dysfunction afterwards, so they are not very much fun and we try not to let them get that far.

One of my colleague’s sons was a college football player who was complaining of back pain. I examined him and thought I could feel a step off between the two lowest spinous processes in his low back. An x-ray of his spine confirmed my suspicion that he had spondylolisthesis at L5-S1.

With spondylolisthesis, a "step-off or "ledge" can sometimes be felt due to the forward displacement of the lumbar spinous process where the vertebra has slid forward.
