Nur de Apotheken haben Gamma-Slim in Kapselform mit seiner einmaligen, geschützten Zusammensetzung, die den sicheren und gesunden Schlank-Erfolg von 3 Kilo pro Woche erst möglich macht.
This is how I understand 'erst" in this sentence:
Only drugstores have Gamm-Slim in capsular form with its unique, protected composition, which provides, for the first time (ever), a safe and sound slimming effect of 3 kilos per week.
That would be another question (into German). You won't find a term in German conveying it exactly but you will have to describe, in German, what "drugstore" means.
That's interesting. No, I am not planning to translate Apotheken as drugstores because the target language is Russian. But what is the problem with drgustores? Since when has the word been banished from usage in the meaning of "pharmacy"?

It's "only" the unique, patented compound of Gamma-Slim in capsule form, available exclusively at pharmacies, that guarantees the safe and healthful weight loss (loss of 3 kilos per week)/ it's only on account of ..., that the safe....is guaranteed/possible.
or: It's only (solely) because of / due to the unique compound...
The German original makes the point that the weight loss only becomes possible because of the unique compound of the product, available exclusively at pharmacies ...
Or, using Kim's phrase: the unique and patented compound is what makes this weight loss possible in the first place (= überhaupt erst)
