Отличие данного способа от предыдущего заключается в первоначальном вскрытии горизонтального полукружного канала с применением защитителя лицевого нерва, предложенного Бурге. Снесение промонтория осуществляется во вторую очередь. Операция начинается при наличии стремени его удалением, после чего в овальное окно вводится шпора защитителя лицевого нерва, который в дальнейшем удерживается ассистентом.
