why something?, why another?, why a second[numerical]?

Explanation:
Biju means 'something' as you said which may imply 'else'. 'Kem' is always why, though depending upon the context 'biju' may mean -
Koi biju avyu che. - Someone else has come.
If Kem is followed by a comma as in Kem, biju? ..It sometimes expresses sarcasm too - So? another [eh?(implied by the tone)].
or Tame biju santru pan khai gaya?- You ate other orange too?
or Pelu biju che te sundar che - That second one is beautiful (neuter gender)

Hope the explanation is helpful.

Pelu is 'that' in this example. Pelu(that) biju(second one)che te sundar(beautiful) che

why another or why the other or why something else?

Explanation:
'Biju' literally means 'second'. 'kem' means 'why'
here it means why something else or why a particular second choice (refering to the context of the topic).
here it is used in context of a query or question or a wonder!
