I would imagine that the document issued by the embassy after supply of all the required documents including this form would presumably be called a PASSENGER DETAILS CERTIFICATE. This is an official certificate that details arrival and departure from Israel (for Israeli passengers only) which can be obtained from Israel through the embassy.

- Self addressed registered envelope.
