R wave amplitude
Explanation:
there is no "exact" translation of the H into E.
commonly used descriptives are:
loss of R wave in the chest leads
decreased amplitude of R wave
poor R wave progression

unofficially, sometimes cardiologists will say - decapitation of the R waves / cut off R waves - but I would not use these terms in a formal translation.
if you can find out what the exact diagnosis is, I may be able to match the correct description to the diagnosis for you.

if the dg is NSTEMI, then one would have to look at the ECG itself, if available, to see if there is:
loss of R wave in the chest leads
or
decreased amplitude.

but from the H, I am going to guess that it should be:
decreased R wave amplitude / height in the chest leads

sorry I cannot be more specific.
