Hebrew term or phrase:יקבלו ליווי של מומחה מקומי
In the glossary I found nearby terms that relate to ליווי by a specialist, but most seem to relate to business guidance.
My sentence relates to an action that will be taken if one develops a certain medical condition; in such case, the local expert will ילווה the patient and advise onwards.
So really I need the most fitting term for ליווי. I thought of "will be followed by a local specialist" but I'm not sure "followed" is the correct term.

many large centers offer treatments under study protocols but cannot provide follow-up when the pt returns to their home. many pts live too far away from the study center. however, the local care-provider (oncologist, cardiologist, etc.) will continue to follow the pt. should a complication develop, or a new problem arise that may be related to the study drug, these professionals usually contact the study center PI and decide whether or not to send the pt back to the study center for more treatment or for evaluation.
in some cases, the local care-provider will even get compensation from the study budget.

I cannot give more info than I have already given. Suffice it to say that the condition is one that requires a specialist who will be able to advise a medical person. Also, I don't really nead a translation for spacialist, I need ליווי here.
It would be helpful to have some idea of the condition and type of specialist as different terminology would be applicable if the specialist in question is a physiotherapist or an occupational therapist or a specialist counsellor or a medical consultant.

Explanation: If it is a social worker or a counsellor, one uses different terms than if it is a specialist medical doctor. Case management is not necessarily a medical function. It can be undertaken by an occupational therapist or specialist nurse. Another possible term you might consider is clinical guidance.
