Indonesian term or phrase: Berfikiran Lembap
I'm working on a back translation; it started off in English, I have been given the Malay/Indonesian translation only and I'm translating that into English, so that the client can double check that the Malay/Indonesian translation was done correctly.

The document is a questionnaire asking a patient to describe their health state. One of the options they have to pick is "Berfikiran Lembap/Cerdas" - I understand Berfikiran Cerdas as in Clear Thinking but have never come across Berfikiran Lembap... All I'm finding for Lembap are references to humidity/moisture/...

Can anyone shine some light on this please?

You're right lembap/lembab refers to humidy/moisture but it also means slow, lethargic and it's not uncommon to describe someone as "lembab" meaning intellectually slow or lethargic.
As for Cerdas, it's actually "bright" or intelligent, rather than clear thinking

Explanation:
Lembap is Malaysian term. Indonesian recognized Lembab, both has the same meaning (damp, moist, humid). But In Malaysia this term has tend to grow extend. I don't know the reason, maybe Malaysian could explain this.
In rough way the meaning of lembap is dull or stupid (slow of mind). From the recite below you could make a conclusion for the term.

Suprisingly, my subconscious mind had been working deal with your question while I am sleeping. It told me the reason Why lembap (moist) grow extend to bodoh (stupid), any porous material that moist is not easy to adsorb liquid due to it has cointain liquid contrary with any porous material that dry, it is very easy to adsorb liquid. Analogy: Moist brain is not easy to adsorb (to capture/to understand) intended mean of an explaination even it's simple explanation. Lembap is more soft than bodoh (stupid) but it could be very sarcastic depend on use thereof.

Anwar Ibrahim said, "Abdullah was choosen by Mahattir, so what he has choosen was wrong".

In other term for lembap in his commentary about Abdullah, he said that Abdullah was "tidak punyai kelayakan" (incapable).
