Thanks guys, I think the person on the certificate (now resident in UK) is getting remarried. I have also been asked to translate his original marriage certificate (from the first marriage) and its annulment. Just a hunch.

Jim: It is all a bit Big Brotherish, but is it any different to an electoral registration office in the UK (except that in the UK, you can opt out)?

When I last moved house the clerk informed me that I should have notified the "ufficio anagrafe" within two weeks and I informed him that I considered that an infringment of my basic human rights. Then he asked me how things worked in the ufficio anagrafe in England. I gleefully explained that we didn't have anything as oppressive as that in England (at least, last I heard, anyway), since it was a total waste of time. This was something I was unfamiliar with and something I just did not WANT to understand.

The certificate is laid out as follows:

Would you call it a certificate of birth/marriage and residence? leave it as it is with a translator's note?

You must know (or maybe you need a refresher stay in the UK) that in the UK and probably also in the US there is no residence certificate, but only birth, marriage and death certificates issued by a single central government office. The only way to translate this is to translate the three lines underneath explaining what it is.
