I'm envisaging those HR logs on which employers record any staff development courses their employees has attended

"have attended" - sorry, I originally said "their staff has attended". That's 1am grammar ;)

I think I've filled these out even just referred to as "Attendance Logs" in the past, or "Staff Development Register" - if this about the staff of the gym of course.

Annaval - considering what you said above, you can replace the word "course" with "class" - for a log that gym goers fill in (assuming this is one of those logs you sign at the gym so they know who's doing what class for insurance purposes - I've filled in one of those too!)
