This is an "Attestato di Rischio" for a vehicle risk certificate. This is a subtitle for a section in the form which goes on to list (in this case) one accident. Obviously I get the idea but do not know the best wording to use in insurance terminology as this area is quite new to me:

My attempt of the complete phrase is:

Accidents taken into account (or "claimed for"??) under the bonus-malus sytem.
