This claims bonus certificate, or "attestato di reschio", has a note under the table listing claims that have been considered for the bonus that reads:

(*)Applicazione del malus o del peius e scattata in quanto le percentuali di responsabilita accumulate nel corso dell'ultimo quinquennio di osservazione della sinistrosita hanno superato il 50%.

This part of the phrase I have tried to translate as:

"...IS COMMENCED IN AS MUCH AS THE ACCUMULATED LIABILITY PERCENTAGE CALCULATIONS..."

...which I know will not make much sense so I would like some help to understand this and find the correct insurance terminology.

Instead of saying "the application of the surcharge has been triggered..." it is simpler just to say "the surcharge has been appplied..."
