Italian term or phrase: attestato di rischio
I know exactly what this is (a certificate of all insurance claims made in a certain period) for the purposes of no claims bonus etc... I just don't know what we call it in UK English.

I think this is the nearest equivalent you will find. You might prefer "no claims bonus certificate" or even "... insurance certificate"

A no claims certificate will very probably give a history of claims over the last few years and in any case must be based on that history. Usually the longer you go without a claim the greater your bonus will be, while if you are on the full bonus and make just claim that will probably reduce your bonus but not eliminate it.
