Medical - Medical (general) / Medical discharge report recommendations.
Italian term or phrase: impegnativa mutualistica
"Prenotare la visita precisando la necessita di eseguire spirometria, per la quale e necessario portare impegnativa mutualistica". I think that it is something from the health insurance company relating to billing. I found "direct billing" for "impegnativa". Suggestions most gratefully received.

Or NHS prescription because that's what it is. A medical prescription which is not issued by a private doctor but by the doctor on a particular form that is created by the NHS. It helps having some costs paid by the NHS.
