Found in an x-ray report: "torace supino: non sono evidenti segni di lesioni parenchimali a focolaio in atto. **Basi libere**. Ombra cardiaco vascolare nei limiti." Not at all sure about that "basi libere", any ideas? Thanks, Sarah.

Explanation:
The above may be a step further from what your Italian original says "lung bases free", but in essence that's it.

With pleural effusion, fluid often builds up in the costophrenic angle (due to gravity). This can push the lung upwards, resulting in "blunting" of costophrenic angle. Shadow on x-ray.

neutral  SJLD: touch of overtranslation there ;-)/have seen "lung bases are clear" loads of times - means nothing in lung parenchyma or pleural space - very non-specific

  -> yes, as I noted in my explanation :), but "free lung bases" alone seems medically incomplete, doesn't it? Though it is actually not our job to "correct" the original.... Perhaps "fluid-free lung bases"
