E’ dunque pronto il vino, e pure la grappa; è tempo di entrare in una di queste case antiche, un tempo abitate da famiglie patriarcali, oggi spesso – contraddistinte da una frasca o da una insegna – aperte all’ospite.
Sullo Zanichelli ho trovato 'bush'quindi ho tradotto con:
'marked by a branch of a bush'ma un parlante inglese capirebbe il senso oppure e` meglio lasciare il termine italiano e dare una spiegazione in nota?

I'm not sure what the problem is...
even in Italian I wouldn't have know that a house with a "frasca" oustide meant that the wine or grappa was ready!!
Just explain it - so the wine, and the grappa, are ready; the time has come to enter one of these old houses, once the homes of patriarchal families, now often open to guests, who will recognise the inn by the leafy bough or sign hung outside.

Explanation:
if you've already translated insegna i doubt you'll find an equivalent
i would translate only one of those terms but you can keep "frasca" too but nobody will understand it
you could translate "sign" and mention the italian term between ( )!
