Italian term or phrase: Italiana d'adozione ormai da tanti anni
"Una vita tra i cavalli quella di Anne Melanie Measures, cittadino britannico (è nata nell’Isola di White), italiana e senese di adozione ormai da tanti anni."

La contrapposizione "by birth"-"By adoption" funzionerebbe molto meglio se non ci fosse quel "ormai da tanti anni". Non riesco a rielaborare la frase in maniera convincente e scorrevole.

"Italian (citizen) since a long time, adopted by (the city of) Siena"

Explanation:
"Italian citizen since a long time" mi piaceperchè semplice, ma escluderebbe Siena;
"now Italian citizen, adopted by Siena"
o un mix delle due: "Italian (citizen) since a long time, adopted by (the city of) Siena"

neutral  Colin Rowe: Surely it means that she has "adopted " Siena, rather than the other way round...

neutral  Kate Chaffer: 'Since' goes with a specific date e.g. since 1980. We need to use 'for' with a duration e.g. for ten years, for a long time.

Italian-Sienese by adoption many years ago

Explanation:
I suggest something like:

"...Measures, British born on the Isle of Wight and Italian-Sienese by adoption many years ago."

if she was adopted in the true sense, I would leave it like this, if you mean that she chose to be adopted (i.e. she chose to move to Siena), I would use "adopted" with the speech marks.

Italy, and more specifically Siena, has been her adopted homeland for many years

Explanation:
I would change the sentence around completely to something along these lines: Although she was born in Britain (on the Isle of Wight), Italy, and more specifically Siena, has been her adopted homeland for many years."
