Explanation:
This is NOT the country of origin.
I have never seen it as one word like this (are you sure this is one word, and not two in two separate lines?), but if iti s one word, it probably refers to the manufacturer who receives the import (the target of the import). I don't know what kind of goods are being imported, but they are perhaps raw materials or other goods that are being used for a manufacturing process in the target country, at this manufacturer.
I think you can call it simply the "importer manufacturer".
輸入先- destination of the import

製造業者 - manufacturer

The word "importer" would be 輸入業者, here 製造業者"manufacturer" makes it more specific, that's why I am guessing the form is not for finished goods for resale but materials for further processing.

Correction:
After more research, I figured that Tomiko is right.
輸入先 is not the destination, target of the import. 先 is used here as the ¥"other party¥" in the import transaction. It is implied that the purpose of the import transaction is to bring the goods to Japan, therefore the ¥"other party¥" of the import transaction is the origin.
So, 輸入先製造業者 is the original manufacturer.

As for your additional questions, 輸入先国 is the country of origin, and 輸入先製造業者名 is the name of the original manufacturer (in the country of origin).
It sounds confusing, but maybe the first line (輸入先製造業者) is a title and under that title you wold need to write the country and the manufacturer to the following two lines.

If you use the manufacturers' country of origin as the translation, it will confuse more. So many companies have factories all over the world. Even though, a manufacturer's country of origin is Japan, but they are made in China at their Chinese factory.

I agree with Tomiko-san’s opinion and knew the rule of the MHWL. If my answer made you confuse what I would like to say. It is not a good English translation for “ 輸入先製造業者“. I meant it for a manufacturer’s name of goods imported from.
I wish Tomiko-san or snowbees-san entered some suggestions.
