agree  Can Altinbay: This one is closer to how you would see it in the US. You might say "persons with...". "Disabled persons" is not wrong, but seems harsher.

would suffice.

Other variations are possible, such as:

"Opportunity conselor for disabled persons"

which puts a positive spin on the title, a popular trend in American business these days.
