華岡青州（江戸時代）は、「療治依頼証札」を患者からとり、診断は告知して、治療中にいかなる不幸な結果がおこっても苦情をいわないことを患者側（治療を依頼する側）に約束させている。

From a history of Japanese medicine and the changing relationship between doctor and patient. I think this illustrates the increased commercialization of medicine in the Edo period, but I'm not entirely sure what 療治依頼証札 means or how best to translate.

One more option for the pronounciation of the source:

"Ryouji-irai(-no)-akashi-fuda"

This one has more results on the web but with okurigana for different terms with 証札. I am not so sure about the pronounciation of 証札without an okurigana 「の」.

- From your description, it seems to be a card (or short letter) requesting treatment from the doctor (= authorizing the doctor to administer treatment).

It's really interesting that they were doing that in the Edo period.
