I only wnated to indicate that commonly it is not called "the reason of the death" as you have stated in your question, but "the cause of death" and I am guessing that you are translating " CAUSE OF DEATH" certificate. There are samples that can easily be found on internet if you google them by these key words. The code in that certificate internationally is selected from ICD-10 code, but I cannot comment more because I do not see the document itself. Hope I helped you a little. |
