I cannot find "A.frik." in any of my income statements, my suggestion must be taken with a pinch of salt (afraid I do not know the exact English phrase -- means roughly "Other tax-exempt reimbursements of expenses").
I may be able to help you more if there is a code attached to this phrase (e.g., on my statement "Forskuddstrekk" has the code 0985).

As for the others:
Gross salary -- correct
Insurance withholdings -- is not, "Forsk.tr." means "Forskuddstrekk", "Advance tax deduction/withholding"
Imposed wittholdings -- "Påleggstrekk" is most commonly translated as "Attachment of earnings", I think
Vacation basis -- Vacation PAY basis?
