Might want to hold off using my suggestion. There are references to what seem to be reports (see http://tinyurl.com/22ru6ca) made to the former Kredittilsynet (now Finanstilsynet) by players in the insurance industry. Perhaps there's a standard reporting form submitted to the regulator involved here?
