Could T possibly mean "terapi"? It looks like a standard phrase used in Norwegian medical records, that was why I asked the question - in case someone had come across it in their work.

The client does not speak Norwegian. She got sick while visiting her daughter and grandchild in Norway and received some treatment there. Now she needs a translation of the documents in order to get a refund from her insurance company, I suppose.
