tantiemy (wynagrodzenie z tytułu praw autorskich) = royalty
opłata z tytułu zarządzania = management fee
należność z tytułu podatków = tax liability
spadkobierca z tytułu pokrewieństwa = heir-at-law , heirs-at-law

Its meaning appears to be "on account of,""due to," or "based on." Then, it is best to find an English equivalent without this added verbiage. But this is my personal opinion.

Here, someone could be double-dipping using another benefit/entitlement or account. The term appears at the end of a line, so here some words need to be added like "based on" and "tytuł" is the reason - entitlement, benefit, etc.
