Na realidade, ao menos am BR PT, a gente pensa no lugar, O contexto, ao menos diz isso -- lugar freqüentado por pessoas refinadas, pessoas de bom gosto. O café é servido no lugar -- logo, a referência ao "café da manhã" mais bem freqüentado. Mas a idéia é o lugar -- o que implica, é claro, que o café da manhã é de qualidade, mas o ponto central é que o lugar é bem freqüentado.
