I'm translating a school transcript from Sao Paulo. There is an entry called "Disciplinas Vestibular" (What's vestivular), and under this entry they have: RED, POR (maybe PDR?). I know these are subjects because it continues with ING, MAT (English, Math), etc. But what's RED, POR? Also, what is Colaçao Grau? (Graduation Date maybe?) Lastly, what does D.O.U. stand for? This is under the entry: RECONHECIMIENTO.

Vestibular is an examination that one should take prior to enter to a University. If an University has more applicants then vacancys, it should promote a "Exame Vestibular" and accept the students with the best grades. A literal translation for "Exame Vestibular" would be "Front Door Examination".
Disciplinas [do exame] vestibular are the subjects of this examination.
RED probabily stands for "Redação" (creative writing) and POR probabily stands for "Português" or "Língua Portuguesa". You are right about ING and MAT. Colação de Grau is the solemnity of graduation and it´s date is considered the graduation date.
D.O.U. stands for Diário Oficial da União, the official journal of the federal government.

University Entry Examination

I hope this explanations will help you:
I have been doing some translation for some Universities in UK and the current term used for vestibular is "Vestibular (University Entry Examination)". I would suggest to put the explanation in parenthesis or as a footnote. About your text specifically I would suggest:
Vestibular Modules: RED(Redacao)=Essay
POR= Portugues and so on as Marcio suggested.
Colacao de Grau is Graduation Ceremony.
Just another note about Vestibular: the vestibular is the Exam (itself) to get a place at the university. In brazil it was developed the preparatory courses to take the vestibular exam when the level of teaching at the Public Schools declined.These course where then called Curso/Cursinho Vestibular and although such courses prepary you better than your seconday school (in same cases) they do not have any academic value in relation to Universities abroad.
