What you're referring to sounds like a "slate", but I don't think that applies here because they're talking about some of registration number belonging to the individual or entity as you note for CNPJ and CPF. There isn't much context because it's on a form.

As far as I can find, it is not an electoral roll number, but can mean a list of candidates for determined positions in an election (ex. president, VP, treasurer, etc.) CNPJ and CPF are the tax registration numbers of legal entities and individuals respectively. Between them, they cover just about everyone, legal and physical! So it is not clear to me, yet, what "chapa" could refer to. Can you give more context? For example, where is this pesquisa to be carried out, in what database? Is this a company document? A bank document?
