Hi all. This is on a Brazilian marriage certificate underneath the "Regime de Bens do Casamento" box which says

"Regime de Bens de casamento: Comunhão Parcial de Bens"

the it says : "Que Passa a Se Chamar: Conserva A Assinatura"

Em inglês, não é necessário forçar o uso da palavra "signature". A expressão "maiden name" (por si só) é de uso universal no contexto.

E certamente deve-se evitar a construção pleaonástica "herein henceforth", sendo "hereinafter" o correto (outra vez, no contexto)

agree  Oliver Simoes: I'm assuming the first part (before the colon) is printed in the document, the second part is to be filled in. I would probably use "named" instead of 'called". She could have decided to use her husband's last name.
