- Acta de presença dos Bombeiros ou, na sua falta, e em incêndios com danos no valor superior a XXX
[etc.]

In UK English, if possible. I gather this is a kind of letter/document from the fire department confirming their presence on the scene, but I cannot confirm this, nor do I know if such a document contains any other relevant information that would need to be included (somehow) in the equivalent term.
