I finally had time to really research this, and it appears that "ccoperado" is sometimes used in Brazil in this context for a physician who is affiliated with something (or someone). However, whem opposed to "credenciado", it is _not_ the member, but someone who is affiliated with a "credentialed" (licensed, or registered) member, such as doctors sharing offices as private partners, or belonging to the same medical organization and sharing facilities. So, while "Cooperado" can be specific, it is pretty broad here, referring basically to someone who may be 'billing' for the procedure based on the law mentioned in the original context. I used "member physicians and other physicians working with them". I am sorry that while some of the answers were indeed "correct" dictionary answers, and we could say that any of them were 'in the right direction' none of them helped me be sure of the basic situation or supplied language that could be used to show that meaning in English in a way that would be understoood by a conceivable target audience. I am grateful for your efforts though.

"duly" qualified, as it implies, however "cooperado" leads to the more ambiguous (cop out maybe) of active or participating - hope it helps

This looks to me like a legal (or contractual) distinction. As such, I think I need to have some idea of a consequential difference between a credentialed (licensed, or perhaps, "official", i.e. registered) affiliate/member, and the "cooperado". Ideally, I would like a clear idea of the meaning of the legalistic distinction (which could, after all, be merely "registered" or the like), and from that I could hope to find out how legal English would term the difference. We may need someone who is familiar with Brazilian legal or contractual terminology (I'm more medical). I do need to be pretty sure that the answer is right in the end, though, so knowing the sources or reasoning for opinions offered would be very helpful.
