Portuguese to English translations [PRO]

I've consulted Portuguese and Portuguese-English dictionaries back to the 19th century. They say that "instrumental" means (besides instrumental) an instrument set, including a set of surgical instruments, and a set of tools more generally. Same in Spanish.
However, this sentence appears to refer to something that is the patient's. I doubt the patient has a risk of problems because surgical instruments could break; I've never heard of such a thing. Could "instrumental" mean more generally "assemblage" or "apparatus" or perhaps a set or linkage of bones? Or does it indeed mean "set of surgical instruments," unlikely though that would seem?

A ref. (1) abaixo traduz a expressão "falência do instrumental" simplesmente como “hardware failure”:
“... Não houve falência do instrumental, lesões neurológicas ou vasculares...”
“...There was no hardware failure, no neurological or vascular injury...”
Além dos instrumentos cirúrgicos propriamente ditos, como os bisturis, a palavra “instrumental” também é usada para designar itens como parafusos e ganchos empregados em cirurgias ortopédicas de coluna. Veja a definição de “instrumental de Cotrel-Dubousset” fornecida pela referência (2). Outras refs. mencionam "instrumental de Harrington". Será um desses o caso?
“O instrumental de Cotrel-Dubousset, desenvolvido em 1980, na França, atendia aos objetivos do tratamento moderno da escoliose(4,5). O sistema é constituído por diferentes tipos de ganchos e parafusos pediculares, que são acoplados por meio de hastes aos elementos posteriores da coluna vertebral...”
