This is from a list of changes that have been introduced in the standardized death certificate :

+ substituição do nome do campo VI, de “Atestado Médico” para “Condições e Causas de Morte”, com o objetivo de afastar, do médico, o entendimento de que **ele seria o responsável, apenas, pelo preenchimento desse campo**

I'm translating the part beginning with "afaster..." as "to disabuse the physician of the impression that..." but the rest of it is confusing me. I've thought of the following possibilities, neither of which makes sense to me:

he/she is responsible **only** for filling out this field
OR:
he/she is responsible **at least** for filling out this field

The interpretation that makes sense to me would be:

"disabuse the physician of the impression that he/she **need only sign his/her name** in this field..." -- but I don't think that's what the text says.

I'd appreciate your comments.

Explanation:
It means that the physician might be required to fill other fields as well. One cannot infer that the other fields will necessarily have to be filled out; e.g., data about pregnancies and childbirth is left blank if the patient is male.

Only one field in the form is signed. The others are data such as the patient's age, sex, cause of death, etc. Therefore, the "disabused" field is probably not the one that requires the physician's signature.

PS. What a horrible chunk of bureaucratese!

Explanation:
The doctors believe only they are allowed to fill out that part of the field. If the name was changed from "Atestado Médico" to "Condições e Causas de Morte", the purpose for doing so was to allow other people to fill in that part of the death certificate and not only the doctor.

Explanation:
I think "campo" means a box or area on a form. I can find examples online but not in Michaelis.
The point is that he/she need only fill in this box on the form.
