Portuguese term or phrase: Pr. Int.
and Pr. Amb.
At the bottom of the report, printed, with spaces to fill in (several of which are not). Is this just "time" or "duration"? i.e. length of time hospitalised and length of time in outpatients?

Explanation:
since there´s no date(s), no number of days, and no time-related note, I´d say --- Prontuário de Internação --- makes more sense than "Período de Internação".
and "Per." would be the most probable abbreviation for "Período".

I would still go with periodo de internação and add a note to the client, I often do that when I translate medical claims full of mysterious abbreviations

Asker: Hi Judith: on the same line are (I think - not very clear) "Conta:" and "C.C." and on next line "Convênio:" followed by the name of a health plan group; d.o.b., gender, Date of admission, weight, etc., follow on next line

Asker: Yes, I think it's probably that; in fact, there's a lot of handwritten stuff too and lots of it is illegible...
