As we have no ID cards here in the UK, I do not know of an exact equivalent. However, I believe that in UK, in legal/public terminology, we would use the term guardianship for the actual matter of chaperoning/caring for the child.

I realise that "card" may not be the exact term required here. If the "Fisa" is just a slip of paper, you could also say "form":
"Guardianship form" etc
(In this case the usage of "form" would not be the commonly used term for job applications or visa requests etc. where you have to provide information, but simply a small piece of paper providing information).

Here is an example of the term, which (as is the case here) can be applied for a true parent, but can also be applied for a non related parent:
"The guardian who is also an Indian visa holder living in India, must therefore grant a written letter of approval to justify grounds for his child’s India visa allocation."
