Помогите, люди добрые!

The title of this international agreement is,
Протокол о внесении изменений в Шанхайскую конвенцию о борьбе с терроризмом, сепаратизмом и экстремизмом, подписанную 15 июня 2001 года в городе Шанхае.

What I have is
Protocol on Amendments to the Shanghai Convention on Combating Terrorism, Separatism, and Extremism that was signed on 15 June 2001 in Shangai.

Of course "signed" in English could mean 1)the Protocol was signed 2) the Amendments were signed or 3) the Convention was signed. Obviously I need to indicate that "signed" refers to "Convention" and not to the others.

Using "which" instead of "that" doesn't seem to help much :(

neutral  Kiwiland Bear: But that doesn't address the problem. In your example there is only one thing (agreement) that can be signed. In the original question there are 3 and the issue is - which one is the word "signed" attached to.

Explanation:
To indicate that "signed" refers to "Protocol" (the former of the two head words in question), you would need a comma just before "signed"

When there's no comma before "signed," the reader will automatically construe it as referring to the latter head word (Convention)

Note:
There obviously should be no 'that is' before "signed" in either case. Otherwise, it would be a "good example of bad style" as a title

One is tempted to use a widespread wording of the form
"Convention of *date* on *subject matter*"
However, it would seem inapropriate to ignore a form of the title that is already widely in use by Chinese media, which reads as follows:
"protocol on revising the Shanghai Convention on Combating Terrorism, Separatism and Extremism signed on June 15, 2001 in Shanghai"

An explanation like "why not" is very convincing, indeed...
However, "being signed" does not exactly indicate whether the signed document is the convention or the protocol, which is what the question is about
Actually, "being signed" can be said about something that is "в процессе подписания" rather than "подписано" (ранее)

disagree  Kiwiland Bear: Excuse me, but: "конвенцию...подписанную" means that it was convention that was signed, not the protocol. And the title used in Chinese media suffers from the same problem as the original question: "protocol on...convention...signed on" - what was signed?
