post-infarction cardiac sclerosis
Explanation:
"Cardiac sclerosis" is a bit more native, but the problem is mostly medical rather than linguistic: this is a diagnosis typical for the ex-USSR health care systems but hardly ever used in the U.S. or elsewhere.

I25.2 - Old myocardial infarction:
Healed myocardial infarction
Past myocardial infarction diagnosed by ECG or other special investigation, but currently presenting no symptoms

I think it would be the most appropriate translation, fully reflecting the source meaning.

The purpose of "постинфарктный кардиосклероз" statement is to point out the presence of the scar.

As for "cardiac sclerosis", it's frequently used to describe the liver connective tissue formation due to "cardiac reasons" (congestive heart failure).

neutral  splotnik: It's mostly about cardiosclerosis (as a result of MI), not MI.

-> "Sclerosis" is a universal definition of connective tissue formation process in a place, where originally it didn't exist. Infarction results in a scar. Nothing else is meant here.
