This expression appears in a doctor's report for a patient who has suffered a head injury:

"На МР томограммах в сагитальной, аксиальной и коронарной проекциях слева по конвекситальной поверхности на уровне висок - темя - затылок определяется патологический МР-сигнал, гиперинтенсивный в Т1 и Т2 ВИ, с признаком наличие уровня."

The patient probably has a sub(?)dural hematoma.

As it became clear from your another question, the hematoma is epidural. So it was a pretty good guess.
