I'm guessing about "circumferential", but "apical region" seems right.

"Circular" is tempting, of course, but "circular apical" seems to pertain to flow pattern in the heart. Other than that, it is more of a botanical term.

Thank you, James! Apical sounds fine but I had to change the rest. It happened to be a myocardial infarction caused by occlusion of the circumflex artery. Mystery solved. :-)
