In modern American usage, I have to say I haven't heard the term "drug den" much at all. "Crack house" is much more common, meaning a house/apartment where drugs are prepared/sold/used. The phrase is commonly in general circles, and by criminals and police. The connotation includes poverty and violence. See the wikipedia article link below.
