- naziv sistema zaštite,
- liène podatke o ugovaraèu osiguranja;
- podatke o motornom vozilu (vrsta, marka, tip i model, broj šasije);
- liène podatke i potpis predstavnika nalogodavca koji saèinjava ponudu i ugovaraèa osiguranja;
- datum i mesto izdavanja uputa za ugradnju kao i otisak peèata Nalogodavca.
