Atrophy would be the more 'medical' term, but the original would then say 'atrofia' http://www.zdravie.sk/sz/p20/458/Slovnik-container.html but that is splitting hairs ...

I dont know if "ubytok" refers to the reduction in size of the muscles or weakening, however I would suggest: patient has reported weak girdle muscles (weak pelvic girdle muscles, weak shoulder girdle muscles...)
