Slovenian to English translations [PRO]

Slovenian term or phrase: Completed her studies at the Faculty of Medicine or graduated from F. of Med.?

Dear all,

I am not an english specialist but I need to translate into english some sentences from the Diploma. It is for my private purposes:

"N.N. je končala študij na medicinski fakulteti in je opravila vse predpisane izpite
Medicinska fakulteta ji zato priznava visokošolsko izobrazbo ter podeljuje naslov doktor medicine"

What is the best translation of the sentences above:

a. ... completed her studies at the Faculty of Medicine/Medical School and has passed all required/obligatory examinations/exams
Faculty of Medicine therefore/so recognizes/is recognizing her higher education and confers/is conferring her title Doctor of Medicine

b. ... graduated from Faculty of Medicine ...
c. ... awards/is awarding/conferring her title ...

d. Something totaly different?

e. the difference between awarding/conferring

f. The next sentences, are they used when you are sending your translated Diploma or not:
name of awarding institution,
name of qualification and title conferred/awarded,
date and place of awarding/conferring

I know it is quite a lot, but I really don´t want to make something wrong here.
