Is this a private doctor - i.e one who works outside of the national institution (at least in the UK!), or have I got the wrong end of the thread?

yes, you're right. It's a private doctor. A patient cannot use a referral from the publicly funded health care system. The patient pays everything out of his/her pocket. I would not use office-based doctor in this case because in some counties they have contracts with HMOs, etc. Your best bet is to simply go with "private doctor" as you suggested.
