From list of roles in which a doctor can be employed - not sure what the British equivalent of this term is.

That's a hard one. I think you can go with the descriptive "office-based doctor participating in the publicly-funded health care network". Anything with the [US] HMOs is not quite the same.

Basically, the physician get's a license from the state to practice in his office ( outside of hospitals and community health centers) and is paid a lump sum *per patient* by the state. Usually, such physicians also have 'private patients ( samoplacniki, your other Q) in order to make a profit. The koncesija is not lucrative per se, it just serves to draw more patients. The sad fact is that most doctors tend to 'exploit' the situation and offer two possibilities to patients:
either you come to me with a referral (and don't pay anything; I have kocesija) on February 2014, or you come as a self-payer (sampoplacnik) next week. The patients select a physician with koncesija, thinking that they won't have to pay, but when they see the waiting period, they decide to become samoplacniki in order to get the appointment faster. Get what I am saying?

Mutual Information System on Social Protection (MISSOC) of the European Commission gives two variants: "private doctor with concession" and "doctor with a concession." Although both translations may be accaptable, I prefer the more accurate "medical doctor with a concession." Some other possibilities: physician with a concession, private physician with a concession.
