Spanish term or phrase: ejercicio
In a description of a survey form, the word "ejercicio" is used for various parts of the form. "Exercise" doesn't do it for me. Aren't the parts of a survey form simply called "sections"?

Sección Dos
Ejercicio 13. Encuesta médica

Exercise would be the appropiate way to translate it when there are different sections within the survey. The exercises or drills are usually within the sections.

Explanation:
Without a context it´s quite difficult.
According to Lozano Irueste: ejercicio del comercio is exercise of commercial activity; ejercicio fiscal is fiscal year; ejercicio normal is normal year; ejercicio presupuestario is budget year and so on. But according to what you say, I think "entries" could be a good word; if that doesn´t suit the context, I would use "sections".

Creo que tienes razón. Hasta donde sé (y trabajé por años en investigación), los cuestionarios y otros instrumentos usados para el levantamiento de datos tienen secciones, no ejercicios.
En inglés hay 'survey exercise', que son prácticas en cursos y cosas así, y lo que se me ocurre ahorita es que tu texto en español quizá venga ya traducido del inglés, pero es sólo una conjetura. Si te sirve de algo (que no creo), estoy tan perpleja como tú ante esos 'ejercicios'.

I think it is the exercise of conducting the survey, as in: "while conducting the survey make sure you follow all the stations...", for example. And this would be equivalent to saying, "while conducting this exercise make sure, bla, bla,..."
"Exercise" may therefore be referring to a procedure.
"Exercise" may also be very much applicable if in the context like "while taking readings, EXERCISE caution to make sure you are using the equipment properly and getting acurate results."
