Spanish term or phrase: notas anexo
This is from the Department of Higher Education, Science, and Technology from the Dominican Republic. I am translating student records from the Autonomous University of Santo Domingo/La Universidad Autonoma de Santo Domingo.

On the first page it states:
"Hacemos constar que la firma que aparece en el record de notas anexo de la carrera de ingeniera civil, expedido a favor de [nombre de la persona] corresponde a [el nombre del director], director de registro de la Universidad Autonoma de Santo Domingo (UASD)"

We put on record that the signature that appears on the _________?

P.S. I am a college student majoring in Spanish education and I am not a professional translator. This is the first legal document that I have had to translate and I am doing it for community service as part of my scholarship.

agree  Helena Chavarria: As far as I know, "notas" = grades, marks, results... It all depends on where you live. Although you could also say "report" instead of "record".
