Rev.Por" might signify "Form reviewed by:". However, I've just checked and "modelo" (somewhat of a leap of the imagination, in any case] means a "blank form", but only in Mexico. And, anyway, it clearly isn't blank, and wouldn't be, if it needed checking. Tauros' medical suggestion seems foreign to the context.
