Juan Jacob: 6:26pm Feb 25, 2004: Really hard, this one... what country could be usefull. Never seen such thing here in Mexico. Por means probably "By", of course. -
Paul Becke (asker): 10:10pm Feb 25, 2004: More specific - Sorry, Juan Jacob. It's Venezuela. Like Juan, I thought maybe "/MdeL. Rev.Por" might signify "Form reviewed by:". However, I've just checked and "modelo" (somewhat of a leap of the imagination, in any case] means a "blank form", but only in Mexico. And, anyway, it clearly isn't blank, and wouldn't be, if it needed checking. Tauros' medical suggestion seems foreign to the context.
