Spanish term or phrase: ficha(s) de visita(s)
Used to record time worked or travelled with respect to a given project. Not sure if these are travel vouchers, or something more general that could also be applied to in-house hours worked??? ("El tiempo utilizado y recorrido que hace durante el mes se registra a través de fichas de visitas")

time sheet
Explanation:
This is the word lawyers use to record time spent with each customer/project. Hope this helps.

Not sure if the travel/expense report works here, because I understand that these forms only registrate time worked or travelled spent in each visit.
