it seems to me that this is a form the subscribing (or ordering since that's next word) client has to fill in... from the context given

maybe the form to be filled in to pay the premium for insurance? Not enough context, just saw this was insurance audit

in finance field it is said subscription form, subscription bulletin; it refers to the application form which is later published. Perhaps this is not the right option in your text. Can you give us the whole paragraph, or more details?
